Dear Provider,
As you might know, Medicare has issued the new Merit Based Incentive Program (MIPS). This is a fundamental change in physician reimbursement, and will permanently alter your fee schedule and the overall well-being of your practice from now on.
Medicare will post your score on CMS.gov, for everyone to see your cost and quality. All insurance companies will follow suit.
We will all have to embark on the journey of fee-for-value, which will eventually replace fee-for-service. Even though this is a complex process, the challenge is not insurmountable. Guardian Health Services is conducting an educational seminar that focuses on tools you will need to prepare you for surviving these incentive payment changes. We will review the fundamental takeaways of the MIPS program, address important MIPS challenges, and show how the Guardian MIPS solutions unlocks your practice’s potential in a clinically integrated transition to MIPS.
The keynote address will be delivered by Michelle Copenhaver, Aetna Regional Southeast Vice President.
Please do not miss this opportunity to join us for this dinner meeting and learn how to elevate your performance, enhance your practice for the highest possible MIPS score, and see how you can become a preferred value-based provider with Aetna commercial and Medicare products.
Your ability to survive and thrive is dependent on your MIPS score –Are you ready?

Winter Park, FL and Boston, Mass., 2/15/2017 – Today, Guardian Health Services announced it is joining CommonWell Health Alliance® as a General Member to help break down barriers to nationwide data exchange. CommonWell, a non-profit trade association, is dedicated to the simple vision that health data should be available regardless of where care occurs.
According to Black Book, 94 percent of America’s providers, health care agencies, patients and payers persist as meaningfully unconnected. In an effort to improve the status quo, CommonWell and its members have committed to using their diverse experience across the health care continuum to produce and deploy real-world, cross-vendor interoperability services that will contribute to improved health care delivery and outcomes for providers and the people they serve.
CommonWell and its members are actively deploying these interoperability services, which are now live at 4,700 provider sites nationwide, ranging from large acute hospitals to rural specialty practices. CommonWell services include person enrollment, record location, patient identification and linking, and data query and retrieval across the health care continuum. Unique to CommonWell, its services aim to solve the patient linking and identity problems at scale. Additionally, CommonWell members have access to a working, functioning record locator service, allowing providers to find health records associated with a patient, no matter where the care is delivered. Services are built into the software by CommonWell member companies to help providers maintain their existing workflow.
“Guardian Health Services shares our vision that health data should be accessible to providers and the people they serve,” said Nick Knowlton, Membership Committee chair for CommonWell Health Alliance and vice president at Brightree. “By adding Guardian Health Services to our diverse membership, we can continue to break down the barriers that have stood in the way of nationwide data exchange.”
Through our innovative HIE based software solution patients, providers, and payors are able to collaborate on a level previously never seen before, as the Guardian Solution is able to constant pull relevant healthcare information from multiple disparate sources and channel them all into a singular Patient Master Chart. By joining CommonWell Health Alliance as a General Member Guardian Health Services can bring even more value to its customers by offering an additional interoperability solutions, increasing the amount and sources of relevant healthcare data in the Patient Master Chart to assist in better identifying and creation actionable items for patients, payors, and providers to act upon.

Tallahassee, FL– The Agency for Health Care Administration (Agency) announced today that Guardian Health Services has joined over 400 hospitals and health care providers as a member of the Patient Look-Up (PLU) network of the Florida Health Information Exchange (Florida HIE).
Guardian Health Services operates Florida Accountable Care Services (FACS), a management and services provider for accountable care organizations. With consent from patients or their designated representatives, the PLU service enables health care organizations and their affiliated users to utilize medical records through a secure connection.
Interim Secretary Justin M. Senior said, “As the Patient Look-Up network continues to expand, each additional member supports the effective use of electronic health record technology by creating a more robust framework. The Florida HIE is an impressive example of health care entities working together for better patient outcomes.”
The Florida HIE links a series of regional networks, or “nodes,” to a larger statewide network. Patient data is stored on secure local servers owned by individual nodes, but is also accessible to members of the PLU service who have the proper authorization from the patient. This structure provides members with the security of local control and the utility of a statewide exchange. Participants are also able to share information with authorized users across the country through the national eHealth Exchange.
The secure exchange of patient information between health care entities allows providers to maintain continuity of care and improve coordination. For example, a primary care provider can request information on a patient’s recent visit to the emergency department at a local hospital. This mitigates the risk of prescribing treatments when there is incomplete patient history and reduces duplicate procedures.
Dr. Sandeep Bajaj, Founder and President of Guardian Health Services said, “Guardian Health Services is excited to join health care partners throughout the state with the mission of improved care coordination. Through the Florida HIE, we will be able to offer our physicians a more complete picture of their patients’ histories, fostering improved care and communication.”
Health care facilities currently using the service include: Bethesda Health, Broward Health, CommunityHealth IT, Florida Hospital/Adventist Health System, Memorial Healthcare System, Orlando Health, Strategic Health Intelligence, Tampa Bay HIE, and UF Health.
